If You Are a Physician…
We make it easy and efficient for you to schedule an appointment. You may simply call us at 562-216-5120 or use our easy fax scheduling referral form attached here. Simply fill out the attached .pdf referral form and fax it with the doctor’s order to 562-733-5880. Our staff will make all necessary contacts to secure the appointment. We will also assist with obtaining prior insurance authorization, if required.

If You Are a Patient…
If someone from your doctor’s office schedules your imaging exam for you, you need not take the following steps. However, if your doctor allows you to schedule your own appointment, we may be asking you for some or all of the information identified here.
When you schedule your appointment with us, it is important to know whether your insurance policy requires a prior authorization for our service. We will obtain the insurance authorization for you so that you do not have to do this yourself. To do this, we will ask you for information from your insurance card and from the order your doctor has given you, so please have both on hand when you call us. If your exam is related to an injury case, we will also require the name and address of your attorney. Once we secure the necessary authorizations, we will call you back to schedule your exam. It usually only takes about 24 hours to obtain authorization from an insurance carrier, though sometimes it can be longer.
LPMI is an approved provider for most major insurance carriers and health plans. We will submit your claim to the insurance company for you. Within 30 days after your exam, you should receive an explanation of benefits (EOB) from your insurance company that identifies the amount of your exam costs that were paid by the insurance company, and the amount that is your responsibility to pay. Once the EOB has been issued on your account, we will send an invoice to you requesting payment of the portion of the service for which you are responsible. Special Clinical Considerations
Imaging procedures are non-invasive, and generally safe. However, if any of the following exist, you must inform us when you make your appointment, as special preparations may be required.
- Surgery to area
- Metallic fragments in eyes
- Surgical or aneurysm clips
- Cardiac pacemaker or heart valves
- Beta blocker
- Silicon or plastic implants
- Prosthesis implant or limb
- Removable dentures
- Implanted cardioverter defibrillator
- Electronic or magnetic implant or device
- Cochclear implant or hearing aid
